PPOs - Preferred Provider Organization Plans

The Shield Select Plan (80/60/50 or 90/70/50)

With a $250, $500, or $1,000 calendar year deductible, the Shield Select Plan gives your covered employees access to our extensive network of providers. With the new no deductible option (80/60/50), members have no deductible when they use Shield Select Providers. When choosing a Shield Select Provider, members:

  • Receive the highest level of benefits
  • Pay a fixed copayment for physician office visits, even if they haven't met their calendar year deductible
  • Are covered for preventive care services, even before they've met their calendar year deductible
  • Are not responsible for provider charges above the amount Blue Shield allows for covered services
  • Rarely have to bother with claim forms or pay for services at the time of their visit, other than the office visit copayment

Members can also choose a physician or hospital from the Blue Shield Preferred Provider Network or a non-network provider. Services received from Preferred Providers and non-network providers are subject to higher copayments, with care received from non-network providers covered at the lowest benefit level. Members must also file claim forms and pay any difference between the amount
Blue Shield allows and the amount billed by a non-network provider.

The Preferred Plan

Our standard PPO Plan is available in a number of plan designs with different deductible and copayment levels, depending on the size of the group. This PPO Plan offers two levels of coverage - one for care provided by Blue Shield Preferred Providers and another for non-network providers. Members receive the highest benefit level when they choose a Preferred Provider, plus they are not required to pay any provider charges above the amount Blue Shield allows for covered services, and they usually do not have to file claim forms.

The Preferred Savings Plan

The Preferred Savings Plan was designed to meet the government's health plan requirements to help small employers qualify to open Medical Savings Accounts (MSAs) for their employees. If you are a small employer, with 50 or fewer employees, your group may qualify for an MSA, an exciting new type of tax-advantage personal investment or savings account intended for medical expenses.

Based on the Preferred Plan 80/50, the Preferred Savings Plan gives members what they want in a PPO health plan - choice, flexibility and access. And it gives employers a comprehensive health plan - with affordable rates - to offer their employees.
